The undersigned Purchaser of dietary supplements agrees to the following:

• Wellness2Gether, LLC is not a licensed provider and does not prescribe or assess micro- and
  macronutrient levels; provide health care, medical or nutrition therapy services; or diagnose, treat or cure
  any disease, condition or other physical or mental ailment of the human body.
• The Purchaser is making health related decisions based on the advice of a licensed health care
  professional (other than Wellness2Gether, LLC) and has discussed any dietary changes or potential dietary
  supplements use with his or her doctor.
• The purchase is made based on advice of a licensed health care provider or based on the Purchaser’s own
  opinion.
• The Purchaser takes full responsibility for their own life and well-being, as well as the lives and well-being
  of the Purchaser’s family and children (where applicable).
• The Purchaser expressly assumes the risks associated with the use of any supplements. The Purchaser releases Wellness2Gether, LLC from any and all liability, damages, causes of action,
  allegations, suits, sums of money, claims and demands whatsoever, in law or equity, which the Purchaser
  ever had, now has or will have in the future against Wellness2Gether, LLC, arising from the Purchaser’s
  past or future use of the purchased supplements.

Welness2Gether, LLC will keep the Purchaser’s information private, and will not share it with any third party unless
compelled to by law.
